Complete your request online, print, sign and date the verification area and mail or FAX it to the NPRC within 20 days as federal law requires all such requests to be in writing.
- Mail or FAX a Standard Form 180 (SF 180): Available at VA offices, Federal information offices or the NPRC. Complete it and mail or FAX it to the NPRC.
- Written Request: You may submit a written request for military records to the NPRC. The more information you can include, the better. Make sure you specify what information, documents or records you are looking for. At a minimum, you should also include identifying information:
- The veteran's complete name used while in service
- Service number(s) and/or social security number if known
- Branch of service
- Dates of service
- Date and place of birth, especially if the service number is not known
If possible, you should also include place of discharge, last unit, duty station or assignment and place of entry into the service. Requests must be dated and signed.
The NPRC tries to prioritize requests so it helps to include the reason for your request and any deadlines that may be related, ie. application for a VA home loan and need to provide proof of military service by a certain date. This should be done in the "Comments" section of the request at eVetRecs, in the "Purpose" section of the SF 180 or in your letter request.
The NPRC tries to respond to requests for DD214s in 10 working days or less. Response times for other records vary greatly, depending on the nature of the request.
You may check the status of your request 10 days after submission by email with your name, request number (if you have one) service number and Social Security number or call the center. (12/06)
